What is Xolremdi used for?

Xolremdi is commonly used to treat WHIM syndrome (warts, hypogammaglobulinemia, infections and myelokathexis).

Xolremdi may also be used for other conditions as determined by your healthcare provider.

How does Xolremdi work (mechanism of action)?

Xolremdi blocks a certain receptor in your immune system called the CXC Chemokine Receptor 4 (CXCR4). This increases the number of certain white blood cells that are moving around in your body. These white blood cells help your immune system fight off infections.

How is Xolremdi supplied (dosage forms)?

Xolremdi is available as 100 mg oral capsules that are taken by mouth.

How should I store Xolremdi?

Xolremdi should be stored in the refrigerator, between 36 F to 46 F (2 C to 8 C). Keep the bottle tightly closed. Store it in the original container and protect it from moisture.

Side Effects

What are the most common side effects of Xolremdi?

The most common side effects of Xolremdi are listed below. Tell your healthcare provider if you have any of these side effects that bother you.

  • Low platelet levels, also called thrombocytopenia (see below)
  • Dry scaly patches of skin
  • Skin rash
  • Runny nose or stuffy nose
  • Nosebleed
  • Vomiting
  • Dizziness

What are the serious side effects of Xolremdi?

While less common, the most serious side effects of Xolremdi are described below, along with what to do if they happen.

Severe Allergic Reactions. Xolremdi may cause allergic reactions, which can be serious. Stop taking Xolremdi and get help right away if you have any of the following symptoms of a serious allergic reaction.

  • Breathing problems or wheezing
  • Racing heart
  • Fever or general ill feeling
  • Swollen lymph nodes
  • Swelling of the face, lips, mouth, tongue, or throat
  • Trouble swallowing or throat tightness
  • Itching, skin rash, or pale red bumps on the skin called hives
  • Nausea or vomiting
  • Dizziness, feeling lightheaded, or fainting
  • Stomach cramps
  • Joint pain

Heart Rhythm Changes. Xolremdi may cause a rare, dangerous heart rhythm problem called QT prolongation and torsade de pointes. Some people have a higher risk of this, including people who are older, have other people in their family who have had these conditions, have low potassium or magnesium levels, or who take some medicines for other heart rhythm problems. Tell your healthcare provider right away if you faint or have changes in your heart rate or rhythm, such as a fast or skipping heartbeat.

Severely Low Platelet Levels. Xolremdi can cause you to have a severely low number of platelets in your blood, also called thrombocytopenia. Platelets help to form blood clots. Having a low level of platelets may increase your risk of bleeding. Tell your healthcare provider if you have any of the following symptoms of low platelet levels.

  • Easy bruising or bleeding
  • Bleeding that does not stop
  • Nosebleeds

Warnings & Precautions

Who should not use Xolremdi?

Allergies to Ingredients. People who are allergic to any of the following should not take Xolremdi.

  • Xolremdi
  • mavorixafor
  • Any of the ingredients in the specific product dispensed

Your pharmacist can tell you all of the ingredients in Xolremdi.

Drug Interactions. Xolremdi should not be taken while you are using certain other medicines. Before taking Xolremdi, tell your healthcare provider about any prescription or over-the-counter (OTC) medicines, vitamins/minerals, herbal products, and other supplements you are using. See the Interactions section for more details.

What should I know about Xolremdi before using it?

Do not take Xolremdi unless it has been prescribed to you by a healthcare provider. Take it as prescribed.

Do not share Xolremdi with other people, even if they have the same condition as you. It may harm them.

Keep Xolremdi out of the reach of children. Since this medicine must be kept in a refrigerator, take special precautions to keep it away from children who also use the refrigerator.

Swallow the capsules whole. Do not open, break, or chew the capsules.

What should I tell my healthcare provider before using Xolremdi?

Tell your healthcare provider about all of your health conditions and any prescription or over-the-counter (OTC) medicines, vitamins/minerals, herbal products, and other supplements you are using. This will help them determine if Xolremdi is right for you.

In particular, make sure that you discuss any of the following.

  • Heart problems or abnormal heart rhythm
  • Kidney problems
  • Liver problems

Other Medicines and Supplements. Xolremdi may interact with other medicines and supplements. Before using Xolremdi, tell your healthcare provider about any prescription or over-the-counter (OTC) medicines, vitamins/minerals, herbal products, and other supplements you are using. See the Interactions section for more details.

Pregnancy. Xolremdi may cause harm to an unborn baby. If you can become pregnant, your healthcare provider should verify you are not pregnant before starting treatment. Use effective birth control while you are taking Xolremdi and for 3 weeks after your last dose. Talk to your healthcare provider about your birth control options. If you become pregnant, stop taking Xolremdi right away and contact your healthcare provider.

Breastfeeding. Do not breastfeed during or for 3 weeks after your last dose of Xolremdi. It is not known if Xolremdi passes into breast milk. Tell your healthcare provider if you are breastfeeding or plan to breastfeed.

Interactions

Does Xolremdi interact with foods or drinks?

Take Xolremdi on an empty stomach after an overnight fast, 30 minutes before food.

Do not eat grapefruit or drink grapefruit juice while taking Xolremdi, because it can increase your risk of having side effects from Xolremdi.

It is unknown if drinking alcohol will affect Xolremdi.

Does Xolremdi interact with other medicines (drug interactions)?

Always tell your healthcare provider about any prescription or over-the-counter (OTC) medicines, vitamins/minerals, herbal products, and other supplements you are using.

In particular, make sure that you discuss if you are using any of the following before taking Xolremdi.

  • Goldenseal, which is a supplement that may be promoted for various conditions
  • St. John’s wort, which is a supplement that may be used to treat depression
  • Digoxin (Lanoxin), which is a medicine used to treat irregular heartbeat and some types of heart failure
  • Metformin (Glucophage), which is a medicine used to treat diabetes
  • Medicines that can cause QT prolongation, which is a potentially dangerous heart rhythm problem

Many other medicines may alter the blood levels of Xolremdi, and vice versa. Tell your healthcare provider about all medicines that you take or have recently taken.

Overdose/Missed Dose

What should I do if I accidentally use too much Xolremdi?

If you or someone else has used too much Xolremdi, get medical help right away, call 911, or contact a Poison Control center at 800-222-1222.

What should I do if I miss a dose of Xolremdi?

If you miss a dose, take it as soon as you remember. If it is almost time for your next dose, skip the missed dose and only take the next dose. Do not take double or extra doses. Do not take more than one Xolremdi dose each day.
